  2. P/U Date

  3. P/U Time

  4. P/U Location.  If PDX, then you must include all of the following:

  5. landing time

  6. airline

  7. flight #

  8. departure city

  9. D/O Location (ex: residence, 123 Main st., Hillsboro, etc.)
